Surveyors also conducted on-site observation interviews with leaders and staff of the organization.

Google 4+ Customer Rating

Handling more than 5.4 billion searches per day, Google is the most used search engine on the World Wide Web. To help their algorithms sort through billions of web pages and provide the most relevant results to their users, Google provides consumers with an opportunity to leave a business review, which includes a rating from one star (poor) to five stars (excellent). Google then averages the ratings to determine an overall star rating.

CMS 5-Star Center Recognition

Our Center has received a Five Star Quality Measures rating from the Centers for Medicare and Medicaid (CMS). The Five-Star Quality Rating System is a tool to help consumers select and compare skilled nursing care centers.  Created by the Centers for Medicare & Medicaid Services in 2008, the rating system uses information from Health Care Surveys (both standard and complaint), Quality Measures, and Staffing. The Nursing Home Compare website features the rating system.

REVIEWS

I was conflicted about leaving a message here, but my brother was under their care and fell during physical therapy the afternoon of 09/09 and was taken to the ER. While I appreciate the immediate phone call, there has been silence on their end since then. That same night I went to pick up a couple things of his as he was taken without any personal items. He has two broken bones and is on the way to have a second surgery. I spoke to the nurse while there and asked that they call me in the morning as it was late as I would like to know what exactly happened and what precautions were taken. They said they would call me as there was a fall report made. I still have yet to receive a call like I was promised. Three business days and a weekend with no call says a lot. Needless to say, I picked up the rest of his items already. Update. Changed it from a 1 to 4 as it did happen with the lack of communication, but they reached out after this and agreed that somehow some way it did get overlooked on by the person who was to reach out. I only hope that they create some policies or checklist in place so that it doesn’t happen again to someone else. They apologized and offered to have the doctor call to explain more of what happened, why, and their suggestions which I appreciated. That’s all I wanted. I just needed to understand what happened, why, and how we can prevent it from happening again. And yes, the doctor did call and even offered some recommendations.
